The LVN Case Manager (CM) addresses the clinical and non-clinical
needs of high-cost and/or high-need members through systematic,
community-based, interdisciplinary, high-touch, and person-centered
care management. The LVN CM primarily works in-person with members,
adults and children, who have chronic health conditions, are
homeless or at risk, have high hospital admissions, substance abuse
issues, behavioral needs, and/or are transitioning from
incarceration. The LVN CM provides case management support for
health education, health coaching, self-management, and transition
of care within the defined scope of practice for a licensed
vocational nurse. This position requires strong interpersonal and
organizational skills to build rapport with members and coordinate
care among healthcare providers and community services. The care
manager collaborates with the member’s interdisciplinary team (ICT)
to support the member and engage their support systems to define
priorities central to the member’s needs and goals. Essential
Duties and Functions: Engage with patients in-person and on the
phone using evidence-based approaches (e.g., Motivational
Interviewing) to promote collaboration with their
medical/behavioral team. Teach and assist members in managing
chronic diseases using SMART (specific, measurable, attainable,
realistic, timely) goals. Collaboratively create and implement
individualized care plans incorporating the member’s medical,
behavioral, dental, and social needs. Participate in
interdisciplinary team (ICT) meetings. Provide support to
non-licensed ECM case managers, as needed. Meet with patients in
the clinic, in their home, or in the community. Provide teaching to
assist with understanding disease knowledge, medication compliance,
specialty services, and other medical needs. Assist members and
their families in solving potential issues related to the
healthcare system, financial or social barriers (e.g.,
interpreters, transportation services, prescription assistance).
Use positive reinforcement and encouragement with a flexible
approach for a diverse population. Advocate on behalf of members
with medical, behavioral, substance abuse, and treatment teams.
Provide community-based resources to members. Complete
Comprehensive Assessments Promote health and work with members on
their provider’s health plan. Thoroughly, accurately, and promptly
document all services rendered in the electronic medical record for
every patient. Follow up with missed appointments, referrals, and
hospital/urgent care discharges. Schedule appointments, send
reminders, and coordinate transportation. Maintain confidentiality
in accordance with clinic policies and HIPAA regulations. Treat
members with respect and work cooperatively with co-workers and
supervisory staff. Provide staff coverage as needed. Requirements:
Education and Experience Requirements: Education: Licensed
Vocational Nurse (LVN) with an active, current, unrestricted
license issued by the State of California. Current CPR
certification. Experience: Two years in clinic service delivery or
managed care environment in the capacity of care coordination role.
Other Requirements: Possession of valid driver’s license and proof
of state required auto liability insurance.
