50% Field Case Manager/ 50% Administrative

Administrative

This is hybrid position with office and field responsibilities that include direct patient care.

  • Will manage the new hospice palliative care patients. Both field visits and tele-health check ins.
  • Will review patient charts at SOC, recertification and concurrently.
  • Clinical support for field team.
  • Take change of condition calls and advise families/facilities regarding the plan of care.
  • Back up coverage for GIP visits as needed.
  • Supervise the aides and RAP programs
  • Provide in-service education to assisted living and SNF as needed.
  • Take field on call as back up as needed.
Field Case Manager
  • Develops and implements a comprehensive plan of care including preventative and rehabilitative nursing procedures where appropriate in accordance with MD orders. Identifies problems and establishes measurable goals
  • Coordinates the plan of care with members of the care provider team and other agencies providing nursing or related services to provide continuity of care and to implement a comprehensive plan of care
  • Performs on-going case management/supervisory visits according to patients needs as ordered by the physician, policy and state regulations to ensure adherence to the plan of care
  • Supervises patient care provided by licensed vocational nurses and paraprofessionals. This supervision includes direct and indirect monitoring of private duty personnel interactions with patients, families, and physicians
  • Coordinates obtaining medical supplies and equipment for patients as needed. Provides instruction to private duty personnel and other care providers on the proper use and maintenance of equipment and medical supplies appropriate for care
  • Provides leadership, the application of new knowledge, and encourages staff motivation. Acts as a resource for patient, families and private duty personnel
  • Participates in the On-Call rotation and as requested

Who you are:
  • Licensed Registered Nurse licensed by the California Board of Registered Nurses and Psychiatric Technician Examiners.
  • At least two years of patient coordination experience or equivalent in home health, hospital or medical group, or Facility..
  • Working knowledge of Medicare Conditions of Participation
  • Current CPR card.
  • Core competency per skills checklist.
  • Current health clearance.
